I’m thinking there are too many small details, and I’ll end up making the design take up most of the shirt.
Any feedback would greatly help.
I like the art the artwork as a starting point and I agree with deaddreamer that it probably needs some color. The placement is fun and printable but I’d say the design needs more to pull the viewer in, maybe some details wrapping around the front area?
add something contraversial to get the viewer’s attention. fresh colours will do, but think about the box. add some random things or animals. add pipelines coming out of the skull with melting koala bears playing on them or whatever twisted ideas may come to your mind. unexpected combinations are pretty interesting.

dbh rarely prints designs that go over the seam – mainly because its quite some additional efford. also i think it does not add anything to your design.
regarding the artwork: maybe some strong color accents could work nicely!
